Important Forms for GDS Compassionate | Download Link

As per the Department of Posts' OM No.17-01/2017-GDS dated 14.06.2023, legal heir of GDS employee deceased  while on duty/service in Post Offices can apply for compassionate employment. To do so, they must submit the GDS compassionate forms through the proper channel (via Sub Division) to the concerned Divisional Head for further processing. All cases will be processed at the Circle Office and must be registered online at

Important Forms

• Application form to intimate the family of the deceased GDS about the scheme of compassionate engagement-Annexure-I
• Application form for compassionate engagement-Annexure-II.
• Unwillingness form-Statement of declaration by the dependents of deceased GDS-Annexure-III.
• Declaration/Undertaking form to be used in case of no candidate is eligible-Annexure-IV.
• Proforma for Sub Divisional Head to send the list of pending cases to Division-Annexure-V.
• Declaration/Undertaking given by Spouse when more than one eligible candidate-Annexure-VI
• Acknowledgement of application for compassionte engagement-Annexure-VII.
• Declaration/Undertaking form for the Applicant-Annexure-VIII.
• Statement of declaration to use handheld devices supplied by Department-Handheld device declaration.
• Statement of declaration for knowledge of riding a two wheeler-Two wheeler riding declaration.
